Problem Statement

Patients in Tanzania move between dispensaries, health centres, district hospitals, and referral hospitals, but their records don't move with them. Facilities rely on paper files or siloed electronic systems, so a new provider often can't quickly see allergies, chronic conditions, current medications, or treatment history. This fragmentation drives repeated diagnostic tests, medication conflicts, incomplete referrals, higher costs, and delays in emergency care while patients have little visibility or control over who accesses their health information

Proposed Solution

AfyaTrust is a blockchain-based access, interoperability, and audit layer for medical records. It doesn't replace hospital information systems or store clinical data on-chain. Each facility keeps records in its existing system, while AfyaTrust provides a shared, trusted layer for locating where a patient's records live, managing authorization, and logging access events. Each patient gets a unique Health ID linked to an approved national identity mechanism, and can authorize access via QR code or PIN

Innovation

The key innovation is combining patient controlled authorization with an auditable emergency break-glass mechanism, balancing privacy with the need for immediate care: exceptional access can be detected and reviewed rather than becoming an invisible, permanent backdoor

Expected Impact

Fewer repeated diagnostic tests and lower patient costs Faster, safer referrals through easily verifiable critical history Quicker clinician access to allergies, chronic conditions, medications, and prior diagnoses Reduced duplicated/fraudulent insurance claims via verifiable records and access trails Stronger accountability through a tamper-evident access history Interoperability without forcing facilities onto one proprietary platform
